Order your repeat prescription or renew a referral online

If you are an existing patient of Rozelle Total Health you can now order a repeat prescription or renewed referral provided you have been seen by a GP at RTH in the last 6 months, and it is a renewal of an existing script or referral. You can also use this form to request a copy of your results provided you have already discussed them with your doctor.

This service costs $30 and will be refunded if we require you to make an appointment with your GP, or if your request is declined.

Quick terms:

• We’ll need up to 3 working days to process your request. If you require your script urgently, please make an appointment to see a doctor.
• If your treating doctor is not available it may take longer.
• Submitting this request does not guarantee a repeat (but you will be refunded if your repeat is declined).
• We can not provide repeats for addictive or restricted medications via this form.
• If you have not heard from us within 3 working days please phone the practice to ensure your request was received.
• The use of this form is bound by our Terms & Conditions, Privacy Policy and Product Policies.
